The aim of this assignment is to identify and examine the role of the Registered Nurse (RN Prior) in the case study below. Students are to identify all the key events that occurred throughout the patient’s admission. Particular emphasis is to be placed on the legal and ethical nursing practice roles and responsibilities.
Mr Rory Symes is a 64 year-old man who underwent a laparoscopic cholecystectomy in a large metropolitan hospital on the morning of 21 May 2017. Mr Symes was transferred to the Extended Day Only Unit (EDOU) at 1430 hours following the procedure. Mr Symes was to stay in the EDOU overnight with discharge planned for the following morning (22 May 2017).

A Medical Officer (MO) assessed Mr Symes at 1630 hours on 21 May due the patient reporting abdominal pain and distension. Mr Symes’ distended abdomen and pain levels were documented in the patient’s medical record by the MO and a phone call was made to report this information to the surgeon who performed the procedure. Analgesia was prescribed (10mg morphine subcutaneously) at 1715 hours to be given PRN 6 hourly in response to the patient’s reports of pain.

Ms Carol Prior was the RN working on the 12-bed EDOU with another RN on 21 May. RN Prior had been registered as a nurse for five years. As per hospital policy, RN Prior and one other RN were the only two staff members rostered to the unit on night-shift that commenced at 2245 hours.

The EDOU was at capacity on the night of 21 May. Two of the other male patients admitted to the unit were distressed; one was continuously vomiting post-appendectomy and the other was experiencing urinary retention following a Transurethral Resection of the Prostate (TURP).

There was also a female patient admitted to the unit who was very upset at being placed in a room with male patients due to her religious beliefs. This patient, who had very limited English language skills, was continually wailing and expressing anger over the fact that hospital management had not resolved this issue as promised to the patient and her husband on the afternoon shift.

At 0210 hours on 22 May 2017, RN Prior documented the following in Mr Symes health record:

‘Temp 38.9, P 126, Resp Rate 16 and BP 110/72 (approx)’

These were the only observations documented during the night- shift. There was no evidence of analgesia administration documented on Mr Symes’ medication chart by the time day-shift staff commenced work.

The day-shift RN for 22 May did report that RN Prior had verbally stated that she had been “extremely busy” all shift and had not finished her “notes” by the time handover occurred.

At 0630 hours on 22 May 2017, Mr Symes’ temperature was recorded by the day-shift RN as being 39 degrees, heart rate 140bpm and blood pressure 80/46mmHg. He was experiencing acute abdominal rebound tenderness and reported that his pain levels had rapidly increased throughout the early morning. Mr Symes was ultimately returned to theatre where a perforated bowel was identified and successfully repaired.

Crystal Hotel Pty Ltd is a privately owned 3.5 stars hotel located in Paramatta CBD in Sydney. The Hotel consists of 160 rooms with maximum capacity of 350 guests, a restaurant with capacity of 150 guests, a function room with maximum capacity of 250 guests and a conference room with maximum capacity of 200 guests. The average price per room per night is $148.
While the hotel is located in a very popular location and close proximity to the Brisbane River and the city centre, it is becoming quite out-dated. The owners rely heavily on their corporate clientele. Clients usually use the hotel for their expat employees. Due to long term contracts they pay on credit with invoices being issued at the end of each month. The hotel is often faced with outstanding invoices. The owners have so far tolerated it as it usually occurs with their long term clients. Additionally to the accommodation services, they often use hotel facilities for their functions and conferences.
The hotel is constantly having difficulties retaining a good quality staff as they always get junior personnel, which once gaining experience will usually leave for better opportunities in hotels with higher ratings. The biggest issue is to retain high quality personnel in the hotel restaurant, especially a chef.
The owners would like to increase the hotel star rating by renovating or refurbishing the hotel and improving their services. They are thinking about building a Wellness Centre on the rooftop of the hotel, which would include a massage treatment room, gym, spa, sauna and an outdoor pool.

DAWG expands
By 2010 there were 15 DAWG shops across Australia. By 2015 the Blooms decided to offer franchises of the business complete with site, shop set up, signage and systems. It took off and the franchise business grew to 40 shops scattered across the nation.

Working for the city
As the business expanded into different and predominantly urban regions, what started out as coastal dwelling dog/pet aficionados who might or might not have kids and who lived in houses with backyards, changed to include inner city dwellers living in apartments and flats. Dogs to this market were often child substitutes who were not just pets but much loved companions who didn’t need schooling and expensive child care and were quite comfortable in a one-bedroom apartment as long as they were well fed and exercised regularly. In the city, smaller breeds were favoured so the DAWG shops stocked little dog coats for little breeds.
The DAWG brand food range remained unchanged, as did the DAWG Salons and services.
This market was growing as the cost of houses was now almost out of the reach of new homebuyers. There were also many new apartment complexes being developed right across the city.

Carl Thomas worked for one of the big outdoor sporting goods stores for more than seven years. Although he never completed his degree, Carl took some management courses at the local community college. The knowledge he gained from his coursework along with his own tenacity enabled him to rise into entry-level management. Although Carl enjoyed his job, he couldn’t help wondering if there was more to life. Carl always wanted to open his own business because he wanted to be his own boss and thought he might be able to earn a decent living.

Recently, retired from a career with the school system as a PE teacher and sports coach, Carl’s Uncle John was looking to fulfill his dream of having an outdoor adventure business. He had inherited some property years back but had not done anything with the land to this point. When Uncle John learned that Carl was thinking along the same lines, he determined it was time to start a business. The two decided to go into business together and brought in Carl’s younger brother, Joe, who was working part-time as an athletic trainer. The trio combined their savings and started hashing out a plan to use the five acres of land that Uncle John had inherited.

The concept was simple…to open a business where teenagers, young adults, and work teams from local businesses could enjoy hours of outdoor fun and entertainment. There was limited sports and entertainment for the target audience so the family decided to open a themed outdoor paint ball park, which they called Outdoor Adventure Paintball Park. Outdoor Adventure offers customers a choice of five battlefields, each offering a different level of play.

Each field provides a unique experience for hours of enjoyment. There is the civil war field with a simulated headquarters and trenches; an old castle, which is made of multiple levels and a tower; the woods, which offers a true woodsy battle with placement of several man-made buildings for additional cover; the village, which is a large field with a wooded section running down one side, a two story building and bunkers in the middle, with a creek running down the other side; and the hill, which contains a wooded section and a number of bunkers on a steep incline. A small store is strategically placed in a location central to the fields to eliminate the need for guests to leave the playing area.

The costs to customers vary, with rental packages starting at $25 per person. Customers may also purchase a la carte based on their individual needs. Additionally, season passes are available for a cost of $150 and birthday party packages are available for $300. The minimum age to participate in a paint ball event is 10 years.

In addition to the five battle fields, there are six air ball fields that are formatted for 3, 5 and 7-man tournament play. Air ball fields offer a variety of layouts that are constantly changed to keep up with the latest craze in tournament play. Many of the fields have dedicated fill stations to eliminate the need for players to leave the field to reload.

The facility also includes a shooting gallery designed to allow individuals to sharpen their shooting skills. The gallery contains high velocity paint guns and a variety of still and moving targets. Players may practice aiming, have shootouts or just blast away at targets for sheer enjoyment.

Carl manages the business and spends most of his time in his office with the door closed, Joe trains new employees and supervises paint ball events, and Uncle John has oversight of the shooting gallery. The business started with three employees but has grown quickly to a staff of 20.

The venture seemed like a good idea. The family’s passion for sports and working with youth appeared to be paying off. There are loyal repeat customers who purchase expensive equipment and supplies from Joe. These customers also enjoy attending extra training and information sessions. The tournaments have become popular and the local news has been covering the events. Moreover, the business has a reputation for being a safe family friendly environment.

However, recently, Outdoor Adventure has been experiencing growing pains. Scheduling is becoming more challenging as the activities on the field increase. Staff is pulled from one area of the park to provide coverage in another. Employees are starting to complain that they do not understand their job duties outside of the paint ball fields and feel they need additional training and procedures. Additionally, a major event was missed due to double-booking. A number of customers have expressed their displeasure with the service and, as a result, spending less time on the field. Local businesses are not responding to special discounts for employee events. There has been an increase in workplace mistakes but fortunately these have not resulted in serious accidents. Customers and employees are starting to question the leadership and often ask, “How long can a business like this one last?” or “Who’s running the show?”

Carl has noticed a dip in sales and is now starting to feel they are losing control of the business. While the two closest competitors are 30 – 45 miles away and do not offer nearly the same amenities, Carl understands that if they do not do something quickly, their customer base may decide travel to the competition. Moreover, his passion for owning a sports-oriented business is waning. He is concerned about the continued success of the business but the work no longer seems fun or interesting.

Uncle John, on the other hand, is not interested in discussing the books and does not see any need to worry. He is not concerned about what he calls “a few random incidents” and sees the dip in sales as an indication that it would be a good idea to expand the offering. In fact, he has been presented with the possibility of forming a paint ball competing team. He feels this opportunity is too big to pass up and wants to convince the others that it’s a good time to pursue.

When planning the AU$65 million upgrade to Crown Towers in 2009, hotel management sought creative ways to differentiate its rooms. Crown wanted to extend its vision beyond remodeled bathrooms and high-end finishes to the actual guest experience. “In-room technology has become a major differentiator in top hotels,” says Ric Lamb, Executive General Manager for Management Information Systems, Crown Melbourne. “Crown decided to equip the rooms with the very latest communications technology to create an even more impressive experience and ‘wow’ factor.”
When originally built, each hotel had its own private branch exchange (PBX) system. Crown wanted to replace the PBX systems with a centralized unified communications system that shared the same Cisco® IP network the hotel uses for business applications. Combining voice and data on the same network would enable hotel guests to use inroom IP phones to view information such as weather, restaurant options, flight information, and more. Crown also wanted to offer a high-quality contact center experience to people who called its reservations center or requested services such as housekeeping or room service.
Solution
After evaluating unified communications solutions from three leading vendors, including the existing PBX provider, Crown chose Cisco Unified Communications. “Many of our guests are businesspeople, and they are already comfortable with Cisco Unified IP phones because they use them at work,” says Mr. Lamb. “The hospitality applications that Crown wanted to provide to guests are available on Cisco Unified IP phones. Crown’s experienced IT staff have the skills to manage Cisco equipment. And we like the idea of having a single point of contact for our network and communications system.”
NetStar Australia, a Cisco Gold Certified Partner, deployed Cisco Unified Communications and integrated it with hospitality applications from FCS Computer Systems, a member of the Cisco Developer Technology Program. So far, NetStar has installed more than 2100 Cisco Unified IP phones in Crown Tower guestrooms and Crown administration offices, and will have installed 7500 when the project is complete. During the transition, Cisco Unified Communications is interoperating with Crown’s original PBX systems.
Each suite has at least two phones, a Cisco Unified IP Phone 7975 with a color touch screen on the desk, and a Cisco Unified IP Phone 7906 in the bedroom. The largest suites have up to 21 IP phones. “Guests can just touch the screen to request room service, call housekeeping, view local weather or weather in their hometown, check flight schedules, view restaurant and retail store information, and more,” says Mr. Lamb. Guests can personalize the phone by saving speed-dial numbers and tagging webpages for quick access. The Crown marketing department worked with NetStar Australia to create attractive menus with a look and feel that reinforces the Crown brand. Phone menus appear in the guest’s preferred language, including Chinese.
NetStar Australia also deployed Cisco Unified Contact Center Express for Crown’s guest services and reservations contact centers. When agents receive a call, they can see the guest’s name and the requested service, such as housekeeping, on their PCs. The same screen displays a staff directory so that agents can just click to transfer the call if needed.
Results
Enhanced Contact Center Experience
In most hotels, calls for extra towels or room service, for example, are routed to different hotel departments. Each department needs enough communications lines and employees to answer all calls during peak calling times. Crown eliminates the guesswork by routing all calls to a centralized contact center with 40 trained agents. Cisco Unified Contact Center Express, which is integrated with the property management software, automatically routes calls to an agent who speaks the language the guest indicated during check in. Contact center managers can generate reports on demand with up-to-the-minute information on performance metrics, including the time needed to meet each customer request.
Streamlined Reservations Process
In the past, Crown’s Reservations Department had to continually check telephones, emails, and the fax machine to process new reservation requests. Now, with Cisco Unity® Unified Messaging, all types of messages appear in one place, the email inbox, saving time for agents and helping them provide better customer service.
Differentiated Guest Experience
Guests have responded positively to the Cisco Unified IP phones, and are especially enthusiastic about the touch screen menus that provide convenient information about hotel restaurants, retail stores, and other attractions and amenities.

Efficient Business Processes
Cisco Unified Communications is also streamlining the hotel’s business processes. For example, when housekeepers finish preparing a room for a new guest, they press buttons on the room phone to indicate that the room is ready. NetStar Australia set up the system so that this information is automatically transferred to the property management system. Immediate updates enable the hotel to make rooms available sooner to guests who arrive early.
Back-office employees like the time-saving features of Cisco Unified Communications, such as the ability to view a list of missed calls, click to call back, and view, playback, and forward voicemail messages from the email inbox.
Simplified IT Infrastructure
Before the upgrade, providing the standard two phones, two televisions, and Internet access in a guest room would have required five cables. “Now voice, television, and Internet access are all provided over the same Cisco IP network, reducing cabling requirements throughout the hotel property,” says Mr. Lamb.
Crown also anticipates reduced operational costs because the hotel no longer needs to use phone technicians to perform telephone extension moves, adds, and changes. Now anyone can move a Cisco Unified IP phone by disconnecting from the old location and connecting it in the new location. This capability is expected to save significant time during set up for conventions.

Next Steps
In mid-2010, Crown will extend Cisco Unified Communications to the Crown Promenade Hotel and Crown Metropol
(open mid-2010). These three hotels will offer guests the highest density of hotel rooms in Australia, and administration offices will share the same servers for IP telephony, the contact center, and unified messaging.
NetStar Australia is currently implementing another application from FCS Communications to further enhance the guest experience. When guests touch a menu option on the Cisco Unified IP phone to request an extra blanket or have their baggage brought to the lobby, for example, the contact center will automatically page the appropriate person. After fulfilling the request, the employee will close the request on an IP phone. If the request is not fulfilled within a defined amount of time, the request will be escalated.

Sleep City previously relied heavily on faxing information. Customer-related email came via head office as the stores did not have their own email addresses. Now Sleep City has introduced email addresses for all stores, slashing fax costs by around 60% and enabling customers to directly contact their local stores for faster service. In February 2006 the company opened a brand new headquarters housing more than 300 administrative, distribution and manufacturing employees. At this site, Sleep City is installing IP video surveillance, enabling managers to check on the facility from any location via the Internet and improving security at a fraction of the cost of CCTV.
Next a Cisco wireless warehouse management system will be introduced with handheld scanners for more efficient inventory management. In future, RFID technology will be used to track stock in real time.
And communication across the growing organisation will be further improved with the launch later this year of a staff intranet. Robert says, “Our employees will be able to download internal forms, such as timesheets and service reports, and shoot them straight to the appropriate person via email.”

Project title: Discrete Element analysis of fibre-reinforced sand

Synopsys:
Short discrete fibres are increasingly prevalent as classical tension resisting elements in geotechnical engineering practice, particularly for improving the mechanical behaviour of weak soils. The technique of soil reinforcement using randomly distributed fibres has been extensively studied in recent years. The fibre-reinforced soil behaves as a composite with a relatively coherent matrix. Fibre reinforcement leads to an increase in the soil strength by interlocking soil particles, improving strain resistance and by preventing from formation of continuous planes of weakness at failure. A body of experimental data exists on the shear strength of fibre-reinforced clay soils examining the stress-strain relationships under monotonic loading. Several investigations have also been targeted towards understanding the mechanical behaviour of fibre-reinforced clay soils using triaxial compression tests, unconfined compression strength tests, ring and direct shear tests and 1D-consolidation tests. A few studies have also been undertaken to evaluate cyclic loading scenarios. In this, a discrete element model will be developed using YADE (www.YADE-dem.org) platform (freeware DEM package) to model the effect of fibres to improve the shear behaviour of clay soils. Therefore, within this model, particle shape, compaction of particles, fibre chains will be modelled using YADE platform in a particulate media using small spheres. The results of the modelling will be validated against available experimental results. The verified model will suggest and estimate the shear strength of fibre-reinforced clay with different fibre contents.

She would clean the entire house room by room, making beds, dusting furniture and woodwork, shaking out rugs. The entire process took a day and a half. When the family was expecting company during the holidays, it was assumed that the house would be cleaned twice, once during the regular weekly cleaning and once the day the company was visiting. When company visited, the living room furniture, which was normally covered with plastic, was uncovered, and the sheet that was laid down on the rug in the living room entrance was picked up and put away. The plastics and the sheet helped to maintain the furniture and rug far beyond the usual life span. This helped the family to save money. Once a year, spring cleaning took place. Everyone in the family, except Myra, dreaded this process. During this event, closets were cleaned out and washed. Hardwood floors were washed and waxed. Woodwork and rugs were washed. Furniture was polished. Curtains were washed and rehung. All the dishes were taken out of the cupboards and washed, and the cupboards were also washed. Then everything was returned to its place. Everything had a place in Myra’s house, and she frequently would become angry if there were too many things out of place. “Family conferences”would be called to discuss ways to help keep the house clean and how hard Myra worked at keeping the house clean while everyone else did so little. Actually, this was a common theme. Myra frequently portrayed herself as a martyr who did so much for others and asked so little for herself. She sees herself as continuously doing for others, when in reality she usually told others what to do.

The yard is also very tidy. The grass is beautifully trimmed and resembles a rug. It is often mowed, even though it does not appear to need it. The garden is well tended; it would be difficult to find a single weed in it. Once when a neighbor cut his grass without a catcher attached to the lawn mower and grasswent flying into her garden, Myra threw a fit. After that, she did not talk to the neighbor for two years.

Now that Myra is retired, the cleaning continues. She always thought that it was her job to care for the house. She was always the one who oversaw the cleaning, although her children or her husband would help. Myra’s mother always took care of their house and thought that it was the woman’s responsibility to do so.

Myra learned how to clean “correctly” from her mother. Both Myra’s parents believed in physical punishment, and Myra and her brother were hit if they did something wrong. Myra was punished if she did not clean something thoroughly enough.

Myra made sure that her family saved money. Some of the activities that she engaged in to save money revolved around cleaning issues, such as the plastic and sheets used to save the furniture and rugs, but Myra saved money invarious other ways as well. Sometimes Myra would wash and reuse aluminum foil. Leftovers were eaten even if they were unappetizing and no one felt like eating them. There was always a concern for money even though the family really was not poor nor lacking in any of the essentials. This was so extreme that Myra would become agitated when someone borrowed 50 cents for a soda and did not return it. Myra would tell stories about relatives who would accept invitations to her house but did not return the invitations. The primary complaint about this was that she was tired of spending money on these occasions without ever getting something in return.

In fact, Myra displayed a disturbing pattern of establishing relationships and then ending them by being rude. She would have an intense relationship with someone she had just started a friendship with, often having that person over for lavish meals and then complaining about some aspect of that person’s personality to Sarah. Myra would sometimes even criticize people to their faces, or she would just stop calling them. Often, these complaints revolved around the idea that Myra was superior to them in some way. For example, someone was too loud and Myra wasn’t; Myra’s house was cleaner; Myra’s cooking was better; and so on. When these people no longer wanted to interact with her, Myra did not understand why.

The family very rarely spent money on anything except the necessities even though they were middle class and could afford it. Thus, they rarely engaged inactivities such as going to the movies, and the children felt as though asking for a new toy was totally unreasonable.

Although Myra is still healthy and she could participate in a number of other productive activities, such as volunteer work, she does not, preferring to spend all her time and energy working on her house and yard. When she visits her grown children, she is dismayed by how messy their homes are, and she begins to clean them. She frequently complains about how other people don’t know how to clean anymore and that her neighbors don’t take care of their property as well as she does.

Public Company Accounting Oversight Board
One of the main features of the Sarbanes–Oxley Act (SOX) is the creation of an independent board that oversees the audits of public companies. Congress’s perception was that auditing firms were not sufficiently independent of the public companies they audited due in part to the audit firms’ receiving sizable nonaudit consulting fees from their audit clients. Thus, SOX created a Public Company Accounting Oversight Board (PCAOB). Public accounting firms that audit financial statements of public companies are required to register with PCAOB and submit to its rules. The board is charged with adopting rules establishing auditing, quality control, ethics, and independence standards. It has the power to regulate the nonaudit services that audit firms may perform for their clients. The PCAOB has the power to inspect periodically public accounting firms and to issue reports of the results of the reviews. The purpose of inspections is to assess the degree of compliance with the requirements of SOX, professional auditing standards, and the rules of the PCAOB and the SEC in the performance of audits and the issuance of audit reports of public companies. In addition, the PCAOB may investigate and discipline audit firms and their partners and employees.
The PCAOB is not a federal agency, but a nonprofit corporation with broad regulatory power like the Financial Industry Regulatory Authority (FINRA), a self-regulatory organization that regulates securities brokers and dealers. It has five members, only two of which may be CPAs. No board member may receive any share of profits or compensation from a public accounting firm.

This comparison will help to identify a community health problem specific to your community. A summary of these data should be no more than one page.

· Windshield survey: Provide a brief summary of the findings from your first assignment. Make sure to discuss elements that link your observations to your identified problem. This should be no more than one or two paragraphs.

· Problem: Using the assessment data, identify the problem that you consider to be a priority concern. Provide a rationale for your choice and relate your choice to one of the Healthy People 2020 specific numbered objectives. Healthy People objectives are located within a topic area under the Objectives page. Your rationale should also include why this is specifically a problem in your community. This should be no more than three paragraphs. Include support of your rationale with at least two scholarly sources such as professional journal articles related to your problem.

· Summary: The summary paragraph of your paper should include a statement about the problem, the population at risk for this problem, and the major direct or indirect factors that contribute to this problem. This information should be no more than one or two paragraphs.

The situation at Good Health Hospital has become a bit overwhelming ever since the outbreak of E. coli in Ward 10 on the second floor. It appears that there are six (6) cases of the disease. To better understand the situation, communication with the CDC has revealed that this particular bacterium can be found in many everyday items. The CDC recommends that investigation begins in the hospital kitchens, since E. coli can be transmitted via contaminated vegetables, as well as delicatessen meats such as salami and other cold cuts. The following table shows the number of cases identified so far:

After meeting yesterday with chief administrator Joe Wellborn, it has been decided that the litigation issues with one (1) of the patients may need further information to determine if the patient was already symptomatic with the bacteria prior to admission.

Research has indicated that hospitals operating in the Tampa Bay area have been known to have cases of E. coli contamination. This substantiates the need for further investigations by the County Health Department.

Due to the severity of the cases, and the potential for further spread, it is recommended that the hospital seek alternate vending services since the present suppliers cannot verify sanitation protocols for their products.

In conclusion, the study done at Good Health Hospital has indicated that the E. coli outbreak was in fact caused by spoil food from the cafeteria.

As a social science researcher working for a regional hospital, you are placed in charge of contacting the DOH from your home state, as well as the CDC for statistical data on nosocomial diseases that frequently cause illness at your health care facility. Using information from both agencies, evaluate the data on common nosocomial diseases and compile a treatment contingency plan for dealing with the hospital infections.

Write a five to six (5-6) page evaluation report in which you:

Analyze Good Health Hospital’s records and itemize recent nosocomial infections that occurred within the past year. In your report, categorize the different parameters (i.e., person, time, place, ethnicity, and gender) used in the compilation of data into the information summative.
Propose at least six (6) questions for the health care administrator at Good Health Hospital, regarding potential litigation issues with infections from the nosocomial diseases. Rationalize, in your report, the logic behind your six (6) questions.
Identify a targeted audience within Good Health Hospital, and prepare an implementation plan based on your hypothetical meeting with the hospital health care administrator. Propose four (4) steps that will be useful in the final implementation plan.
Suggest at least five (5) recommendations to your department head based on the steps taken in the implementation plan. Provide rationale for your suggestions.
Using these approved recommendations, design a safety protocol itinerary that must be placed in public access areas of the hospital.
